Download Records Release Form.  You must submit this form signed by a parent prior to any records being released.

Download Senior Year Questionnaire.  This form provides us with information about you that we use for recommendation writing along with an autobiography and resume.

Download Self-evaluation Statement .  This form is also used for recommendation writing.

Download Teacher Evaluation Summary .  This form is given to your counselor by a teacher you select and is used when we write recommendations.  This is not the teacher recommendation to be sent to colleges.

Download Parent Brag Sheet .  This form is used by your counselor to get another perspective about you for writing recommendations.

Fill Out On-line application notice.  This form is to be submitted to your counselor when you have done a college application on-line. Please also turn in any additional forms the counselor must fill out and an addressed envelope.
