Records Release Form	You must submit this form signed by a parent prior to any records being released.
Senior Year Questionnaire	This form provides us with information about you that we use for recommendation writing along with an autobiography and resume.
Self-evaluation Statement	This form is also used for recommendation writing.
Teacher Evaluation Summary	This form is given to your counselor by a teacher you select and is used when we write recommendations. This is not the teacher recommendation to be sent to colleges.
Parent Brag Sheet	This form is used by your counselor to get another perspective about you for writing recommendations.
On-line application notice	This form is to be submitted to your counselor when you have done a college application on-line. Please also turn in any additional forms the counselor must fill out and an addressed envelope.
